SPA ONE TATTOO REMOVAL UNLIMITED MONTHLY PLAN

By enrolling in the Spa One Unlimited Monthly Plan, you agree to the following terms and conditions and receive the following benefits.

1. Medical Oversight

Treatments are performed by a trained master esthetician or nurse under the direction of Dr. Paul Benson, MD., Spa One's medical director.

2. Enrollment

Plan and benefits begin the moment you enroll. This includes savings up to 50% on tattoo removal and complimentary treatments, FREE doctor clearance ($50 value), FREE prescription and supplies ($50 value), and FREE rewards points for select products and services. 

3. Monthly Plan Price

The monthly plan price is based on tattoo size. The plan applies only to the tattoo area approved and documented during enrollment. XX-Large tattoos, unusually complex tattoos, or multiple tattoo areas require a custom quote.

4. Photo Documentation

You agree to allow Spa One to photograph the tattoo for documentation, treatment planning, progress tracking, and medical purposes. Photos will not be used for marketing, advertising, social media, or public display without your express written consent.

5. Treatment Schedule

Tattoo removal requires time between sessions for the skin to heal and for the body to clear ink particles. Allow 6-8 weeks between treatments, subject to skin response, provider recommendation, and medical clearance.

6. Unlimited Treatments

The plan includes ongoing medically appropriate PicoFy treatments for the enrolled tattoo area while the plan remains active. Partial removal or fading for cover-up preparation commonly requires approximately 3–5 treatments. Complete removal commonly requires approximately 8–12 treatments or more. These are estimates (not guarantees) based on extensive manufacturer research.  

7. No Guaranteed Results

Spa One does not guarantee complete removal, a specific level of fading, or a specific number of treatments. Results depend on many factors, including tattoo color, ink depth, ink density, tattoo age, location, skin type, immune response, sun exposure, and aftercare compliance.

8. Numbing Cream

Prescription-grade topical numbing cream is included and may be mailed by the pharmacy directly to you directly or to Spa One for you. Numbing cream is optional, and you may decline its use. You agree to use numbing cream only as directed and to follow all instructions provided by Spa One, the pharmacy, and the  manufacturer. Do not use numbing cream before driving or operating machinery. Keep numbing cream away from children and pets. Any unused prescriptions should be disposed of according to pharmacy or medical disposal instructions. 

9. Adverse Reactions

Adverse reactions are rare but possible. Common temporary effects may include discomfort, redness, swelling, tenderness, frosting, blistering, scabbing, bruising, itching, pinpoint bleeding, or temporary skin color changes. Less common risks include burns, infection, scarring, permanent pigment changes, incomplete removal, or darkening of certain pigments. You agree to notify Spa One of any unusual reaction or concern.

10. Sun Exposure Notice

You must avoid sun exposure, tanning beds, and self-tanner on the treatment area before and after treatment. Treatments may be postponed if the skin is sunburned, recently tanned, irritated, infected, wounded, or otherwise unsafe to treat.

11. Aftercare Instructions

You agree to follow all pre-treatment and post-treatment written instructions. This may include keeping the area clean, avoiding picking or scratching, protecting the area from sun exposure, avoiding pools, hot tubs, and public baths, and keeping the area moisturized with Aquaphor or an approved alternative. You agree to notify Spa One of any unusual reaction or concern.

12. Medical Eligibility

Treatment may be delayed or declined based on medical history, medications, pregnancy or breastfeeding status, skin condition, recent procedures, infection, sun exposure, or provider/medical director judgment.

13. Payment Timing & Notifications

The first payments is due when you enroll. Subsequent monthly charges will continue approximately every 30 days until cancelled. Spa One will send a courtesy reminder before attempting to process your payment each month. Payment issues may result in suspension or cancellation of the plan.

14. No Refunds

All fees and payments are non-refundable. Spa One does not offer refunds or credits for unused treatments, missed treatments, partial results, discontinued plans, or failure to follow treatment instructions.

15. No Interest or Penalties

Spa One does not charge interest or penalties as a part of this plan.

16. Cancellation

You may cancel anytime without cancellation fees. All plan benefits end immediately upon cancellation. If you decided to re-enroll, you must pay an re-activation fee of $100.

17. Provider Discretion

All treatments are subject to Spa One discretion. Spa One may refuse, delay, modify, or discontinue treatment when necessary for client safety, medical appropriateness, policy compliance, or professional judgment.

18. Spa Policies

You agree to follow all Spa One policies, including cancellation, no-show, illness, refund, discount, membership, children, pet, and safety policies. You are responsible for any applicable same-day cancellation, no-show, late-cancellation, or other fees incurred under Spa One policy. Failure to follow spa policies or pay any fees when due may result in suspension or cancellation of the plan and loss of plan benefits.

19. Plan Changes or Discontinuation

Spa One reserves the right to modify, pause, or discontinue the PicoFy Tattoo Removal Unlimited Monthly Plan at any time with written notice to you. If the plan is discontinued by Spa One, any unused prepaid monthly plan payments will be refunded in accordance with Spa One policy and Utah law. Spa One may immediately suspend or cancel the plan for nonpayment, safety concerns, medical ineligibility, misuse of the plan, or failure to follow Spa One policies.

20. Entire Agreement

These Terms & Conditions, together with any written enrollment form or written pricing disclosure provided by Spa One, represent the entire agreement between you and Spa One for the Spa One Tattoo Removal Unlimited Monthly Plan. No verbal statement, promise, estimate, or representation will modify these terms unless confirmed in writing by Spa One.

21. Written Terms Control

If any verbal communication, text message, email, advertisement, website statement, or other communication conflicts with these Terms & Conditions, these Terms & Conditions will control, unless Spa One expressly agrees otherwise in writing. 

22. Severability
If any part of these Terms & Conditions is found to be invalid, unenforceable, or unlawful, the remaining terms will remain in full force and effect.

23. No Waiver
Spa One’s decision not to enforce any term at any time does not waive Spa One’s right to enforce that term in the future.

24. Governing Law
These Terms & Conditions are governed by the laws of the State of Utah.

25. Client Acknowledgment
By enrolling, signing, initialing, electronically accepting, or making payment toward this plan, you acknowledge that you have read, understand, and agree to these Terms & Conditions.
